RESPONSE TO YOUR REQUEST

While our aim is to provide information whenever possible, we regret to inform you that our institution does not maintain the data you are requesting on COVID vaccination status of cancer patients. This data may possibly be obtained by reviewing patient health records, however, this will require direct coordination with our Clinical Research Department. You may reach them at crd@pcmc.gov.ph.

Please see the attached response from the Clinical Research Department and Cancer and Hematology Division regarding your request.

YOUR RIGHT TO REQUEST A REVIEW

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Ms. Joi Pangilinan at fdmdpo@pcmc.gov.ph. Your review request should explain why you are dissatisfied with this response and should be made within 15 calendar days from the date when you received this letter.
