Good day!

Please provide raw data without PII of cancer patients in your hospital on the following on a monthly basis:

1. Vaccination status/date of vaccination for covid-19.
2. Age.
3. Kind and stage of cancer. 
4. Onset of cancer.

RESPONSE TO YOUR REQUEST

Due to the nature of the data you are requesting, please submit a formal letter of request to our Executive Director, Dr. Sonia B. Gonzalez, MD, MScHSM, MPM, detailing the raw data being requested and containing a specified purpose/research. You may send your letter to officeofthedirector@pcmc.gov.ph via email or to Philippine Children's Medical Center, Quezon Ave. corner Senator Miriam P. Defensor-Santiago Ave., Quezon City.

However, we are informing you that our institution currently does not maintain data on the COVID-19 vaccination status of our cancer patients. 

YOUR RIGHT TO REQUEST A REVIEW

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Ms. JOI PANGILINAN at fdmdpo@pcmc.gov.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
