August 14, 2023 Dear Sofia May Loresco, Greetings! Thank you for your request dated Aug 12, 2023 12:50:59 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. Your request You asked for PSBank Application Forms. Response to your request Allow us first to introduce to you the Philippine Deposit Insurance Corporation or PDIC: PDIC is a government instrumentality created in 1963 by Republic Act 3591, as amended, to insure the deposits of all banks. PDIC exists to protect depositors by providing deposit insurance coverage for the depositing public and help promote financial stability. PDIC is tasked to strengthen the mandatory deposit insurance coverage system to generate, preserve, maintain faith and confidence in the country's banking system; and protect it from illegal schemes and machinations. Going back to your request for various Philippine Savings Bank (PS Bank) application forms, while PDIC provides deposit insurance to all banks in the Philippines, we regret that being a deposit insurer does not entail having in its possession the forms being used by banks in its operations. May we instead suggest a personal visit at any branch of the PS Bank. We took note that you are a student of the Polytechnic University of the Philippines. As such, please find attached the list of PS Bank branch within the National Capital Region with their respective addresses. May we also seek your help to improve our service delivery by answering the attached survey feedback. There is no right or wrong answer.
