Good day! I hope this request finds you well and safe amidst the transition of the public health emergency. 

We would like to request for any record on the Republic Act of the creation (1923) and or expansion of the Tarlac Provincial Hospital in San Vicente, Tarlac City. 

The document if provided will be used for the requisition of this goverment hospital to be a training provider under the Professional Regulations Commission.
