Your Request

You asked for New Application for Water Connection (Application for Water Service Connection and Installation Order Form).

Response to Request

Your FOI request is approved. I enclose a copy of the information you requested. 
However, please be advised to return the accomplished application form together with the following thru our email address at tandag_water_district@yahoo.com.ph:
         1. Barangay Clearance; and
         2. Building Permit, for newly constructed building/house.

Also, a payment fee of P3,400.00 for the water meter and its installation shall be made at the TCWD Office, Brgy. Mabua, Tandag City.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to WINIFREDA A.
