This refers to your request dated 12 May 2023 under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch. 
 
You asked for information on the number of BS Nutrition undergraduate students who have already taken HNF 122.

Since you did not provide Form F which needs your signature and should be administered under oath as required under the UP FOI Manual, we regret that we are constrained to DENY your request.

Article III Section 7 of the 1987 Constitution, which is the basis for the requirement under the UP FOI Manual, states:

“The right of the people to information on matters of public concern shall be recognized.
