1. Copy of the BOR approval on the list of graduates of batch S.Y 2017-2018, 2018-2019 & 2019-2020.
2. Certified True Copy of the institutional policy of University of Southeastern Philippines as basis on the declared equivalent academic year and term and/or in clarification of the nature of thesis/dissertation writing or its equivalent academic requirement.
