1. Copy of the BOR approval on the list of graduates of batch S.Y 2020-2021 & 2021-2022.
2. Certified True Copy of the institutional policy of University of Southeastern Philippines as basis on the declared equivalent academic year and term and/or in clarification of the nature of thesis/dissertation writing or its equivalent academic requirement.
