Hi! I would like ask if your office has forwarded the Vaccination Records from the dates of July to August to Department of Health or to any concern goverment offices because I'm requesting my VaxCertPH certificate  and my records were not located from their side despite constantly following up to my local government unit and to VaxCertPH website for 3 weeks about my vaccination records.

Sir we have forwarded your record to the Quezon City Health Department last December 29, 2021. The concerned office has verified the request and your record first especially with the brand of shot given to you. The Quezon City Health Department will then forward it to the Department of Health.
