Your request has been denied due to your request was lodged in the wrong agency.

While our aim is to provide information whenever possible, in this instance this Office does not operate the updating of Vaccination Certificate.  However, you may wish to contact the Department of Health at vaxcertsupport@doh.gov.ph, or resubmit your request addressed to the correct agency on the eFOI portal at https://www.foi.gov.ph/requests?agency=DOH.

Your right to request a review
