Dear Regine Almazon,

Greetings!

Thank you for your request dated Nov 03, 2020 07:20:06 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request to get a copy of your Transcript of records was immediately coordinated to the Registrar's Office for their appropriate action. But unfortunately, upon checking your student account, they found out that you have a balances to your tuition fee amounting to Php 5,230.00. 

In this regard, kindly settled the above-mentioned balances to clear your clearance in the campus.

If any inquires kindly coordinate to the Registrar's Office at (043) 778-2170 local 110 and Accounting Office at (043) 778-2170 local 105 for the settlement of the said balances.
