Thank you for your request dated Jun 05, 2023 01:27:58 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request

You asked for Transcript of Records.

Response to Request

You may personally visit the Registrar’s Office from Monday to Friday 8:00 AM to 4:00 PM to request the needed documents. 
For your TOR request, proceed first to the Registrar's Office window, accomplish the clearance form and get the payment slip and pay the amount due at the Cashier's Office. Present the duly accomplished clearance form and official receipt to the Registrar window and get the claim stub. Bring 1 pc. documentary stamp.
All requested documents are processed within 7 to 10 working days upon receipt of payment. 
An authorization letter, ID of the student and ID of the representative must be presented if the concerned student is not available to receive the requested document on the scheduled date of release.

Your FOI request is approved. I enclose a copy of [some/most/all] of the information you requested.

Your right to request a review

If you are unhappy with this response to your FOI request, you may ask us to carry out an internal review of the response by writing to Merirose M. Puno/merirose.musca@bulsu.edu.ph. Your review request should explain why you are dissatisfied with this response, and should be made within 15 calendar days from the date when you received this letter. We will complete the review and tell you the result within 30 calendar days from the date when we receive your review request.

If you are not satisfied with the result of the review, you then have the right to appeal to the Office of the President under Administrative Order No. 22 (s. 2011).
