Hi Ma'am/Sir:

We would just like to inquire if there is a certain water district in Zambales that had a blacklisted contractor named, "AQUATERRA KONSTRUCT, INC.". If so, may we get a copy of the certification that the said contractor is blacklisted, also, stating the reason why it became blacklisted?

This information is important for BAC purposes.
