Hello I would like to request a physical form of my form 137 I was studying in Mindanao/ surigao city way back 1994-1997 I coming here in ncr after I graduate without those document, not to think that it is very important for now' while I'm applying back to abroad I'm living here in manila, and it takes for me big cost for traveling back again to Mindanao to take this form..and I have no time the company needs this paper as soon as it is available.thank you very much I hope this request granted' God bless..

You may refer to the Guidelines on Learner's School Records in the DepEd Order No. 54, s. 2016 titled "Guidelines on the Request and Transfer of Learner’s School Records" regarding your request for a Form-137 (Permanent Record), Form-138 (Report Card), and other school records for your information and guidance:
