Hi! I would like to request another vaccine card because mine got lost. I don't have any pictures of my vaccine card. However, I remember when and where I got vaccinated. I got my first dose on May 25, 2021 in Central Philippine University and my second dose on June 24, 2021 at the same place under the name Albert Glor B. Supapo. I hope my request will be processed.
