This is to kindly request a copy of the following documents, if applicable. For your reference, I have coordinated this request with Mr. Hubert Guamos of the OED.

1. Productivity rating report (OPCR) of the Mergers and Acquisitions Office (MAO) for years 2018, 2019, 2020, and 2021;
2. Raw or processed data on CSS feedback forms for MAO (if raw feedback forms - at least 15 forms from 2017 to 2019 and at least 15 forms from 2020 to present); and
3. Copy of letter from the Office of the Executive Director to the Civil Service Commission regarding work offsite policy (pre-pandemic).
