5.) Duration of treatment/therapy
6.) Successful(patient fully recovered?)/unsuccessful --this is a binary outcome
7.) Medicines prescribed(if any)
8.) Age of patient
9.) Gender of patient
10.) Occupation/nature of living(e.g. athlete, soldier)
11.) Home address of patient

While our aim is to provide information whenever possible, in this instance PCHRD does not have the information you have requested. However, you may wish to contact UST Hospital-Apolinario Mabini Rehabilitation Center at (02) 731 3001 local 2432 / 2297, a rehabilitation center we have collaboration with.
