Greetings!

Thank you for your request dated Aug 18, 2023 12:06:58 PM under Executive Order No. 2 (s. 2016) on Freedom of Information in the Executive Branch.

Your Request on the reissue of your vaccine card, it can be requested from the Municipal Health Office where you have been vaccinated.
