To facilitate the processing of your request for your company's Certificate of No/Pending Case with the NLRC, may we refer to you the proper procedure for applying for the issuance of NLRC Clearance / Certificate of Pending/No Pending Case:
Checklist Requirements:
1. Letter - Request in three (3) original copies and written in company letterhead of requesting party addressed to :
A. FOR ISSUANCE OF NLRC CLEARANCE/CERTIFICATE OF PENDING/NO PENDING CASE ON APPEAL :
ACTING EXECUTIVE CLERK OF COURT IV MONA LISA M. VARGAS, National Labor Relations Commission 7th Floor, Ben-Lor Bldg., 1184 Quezon Avenue, Quezon City
B. FOR ISSUANCE OF NLRC CLEARANCE/CERTIFICATE OF PENDING/NO PENDING CASE (NCR ARBITRATION BRANCH) :
EXECUTIVE LABOR ARBITER JENNETH B. NAPIZA, National Labor Relations Commission-NCRAB, 6th Floor, Ben-Lor Bldg., 1184 Quezon Avenue, Quezon City
2. One (1) copy of Authorization, giving authority to the person who will pick up the clearance/certificate with attached one (1) valid Government ID of authorized individual and one (1) valid Government ID of the requesting individual.
3. One (1) copy of Affidavit of Undertaking of no Pending Case on Appeal or Self Certification of No Pending Case
4. Payment of fees in the total amount of P530.00 (P500.00 for NLRC Clearance and P30.00 documentary stamp tax)
5. Pre-paid return pouch attached to the Letter-Request for Certificate of No Pending Case if requesting party is stationed outside of Requesting Party Requesting Party Requesting Party Requesting Party Requesting Party (Courier or Philippine Postal)
Submit the complete required documents to the Receiving Officer / PAC (1st Floor, Ben-Lor Bldg., 1184 Quezon Avenue, Quezon City)
