Hi! I would like to request copies of my form 138/ Report Card (Grade 9 & 10) or Form 137/TOR. It is required po for my college admission.  Yung school ko po nung grade 9-10 ay  Tungao National High School S.Y. 2016-2017 at Bagbag National High School SY 2017-2018 naman po nung grade 10. Nagtry na po kami mag request sa school pero wala na raw po silang copy ng report card. Tinry na rin po namin mag request ng Form 137 sa SHS ko pero di raw po nila ito mabibigay kasi need ng request form. salamat po in advance!
