Hi, 

I am writing to Makati City hall and was wondering if there is a kind of standard or template letter to request data under the freedom of information act? Can someone provide me with a letter template to write to government officials and how they should be addressed? Thank you!

The Freedom of Information do not have jurisdiction over the LGUs. You may check directly with Makati City if they have FOI ordinance. If they have ordinance, they have FOI template for your to lodge a request to their municipality.
