You asked for Lost vaccination card.

Response to your request

While our aim is to provide information whenever possible, in this instance this Office does not have  the information you have requested. However, you may coordinate with the LGU where you received your vaccination as their office shall verify your records prior to the issuance of a new vaccination card.
