I would like to to request for this documents onmy intent to go back to college 

1. Form 138/ReportCard
2. Certificate of GoodMoral

I plan to be a working student but im lacking of this documents. I was able to submit it at pangasinan state university (lingayen campus) I initially planned to proceed in college however my father had an accident so I need to stop to work for my family. I finished only my first semester and Im sincerely sorry for my negligence
