Greetings!

Hi, I would like to request for an authorization letter coming from your respected office stating that we are given the consent or authority to use a lot owned by your office. The said document request will be used to apply for electrical installation.

It is best to include the following details: 

a) Lot number 2528-H located at P5 Cale Tiwi Albay. 
b)  It would be best to include the name of Mrs. Marife C. Dula, the current resident of the said lot and has been occupied since 2001.
