This is to respectfully request a certified true copy of the Cadastral Map of Cadastral Lot No. 304, CAD-434-D, covering a parcel of land situated in Int. San Bartolome St., Brgy. D-1. City of Borongan, Eastern Samar.

This request is in view of the preparations for the filing of the petition for registration of title for the said parcel of land.
