Hi. I would like to request for Information regarding randomly receiving a FORM 1525 addressed to Louise Castro with the address of Blk 19 Lot 12 gaineswood drive marquee place subdivision.

Response to your request

We were able to determine that this particular information is not a valid FOI request as the same is not a request for a document, but is more of a customer service or front line service request which should be addressed to and could be best answered by customer service. 

Nevertheless, please be informed that said form is issued in order to allow you to claim a registered letter that could not be delivered to you. You should coordinate with the local post office within the prescribed period stated therein in order for said it not be returned to sender.
