Good day,

I have a SSS online account but i forgot my userID and password unfortunately. I also forgot my email account that i used in registering in My. SSS.

May i request that my email address be replaced with [raffyfranciscotorculas@gmail.com] which is my active email as of date.

Here are my details relevant to this purpose:

Name: Torculas, Raffy Francisco.

Date of birth: August 18,1999

SSS/CRN number: 0111-9737419-8

I've also attached two copies of my valid ID as proof of my identity.
